Brazilian Butt Lift – The Surgery
Excess fat is removed by liposuction from any areas you choose, and then separated from the liposuction fluid in a closed sterile system with no further manipulation of the fat. The healthy, pure fat is then injected carefully and in multiple different layers as microdroplets into the butt area for a fuller, more lifted and younger looking buttocks. The process of fat reinjection involves several passes of injections through a few tiny incisions in the skin. The procedure is designed to fill the upper and lower quadrant of your buttocks as well as your lateral hips and upper posterior thigh so that the butt appears lifted and perky. The resulting effect is that your profile appears more attractive and sensuous.
Liposuction is performed through small, inconspicuous incisions using the water assisted liposuction (WAL – BodyJet®) technique. First, diluted local anesthesia is infused to reduce bleeding and trauma. Then a thin hollow tube, or cannula, is inserted through the incisions to loosen excess fat using a controlled back and forth motion. Using the so-called “Water-jet system”, liposuction occurs gently, avoiding damage to the fat cells which is vital to the success of the surgery. The dislodged fat is then suctioned out of the body using a surgical vacuum attached to the cannula. In general, we liposuction the maximum amount of fat that is safe, which will then be transferred into the buttocks and hips into different layers.
Water-jet assisted fat transplantation is efficient and gentle to the fat cells and eliminates the need for elaborate centrifugation and any other preparation of the fatty tissue. The fat or lipo-aspirate that was harvested through liposuction is then separated from the remaining fluid in a special sterile container to get pure fat with healthy adipocytes (fat cells) and stem cells that can be immediately used for water-jet assisted fat transfer.
Dr. Stasch will use a small injection cannula to add the fat to the buttocks and hips through small injections in previously carefully determined areas of your buttocks / hips. The crucial surgical skill is the proper positioning of the fat in safe layers of your soft tissue. Dr. Stasch will pay meticulous attention not to touch the muscles, which are a no-go zone for fat grafting in the buttocks. That way he can guarantee your safety and still achieve amazing results.
What Is The Downtime After My Brazilian Butt Lift?
While you can expect some soreness and swelling, many of our patients resume daily activities after 1-3 days following their Brazilian Butt Augmentation procedure. You should avoid sitting and lying on the areas of fat grafting for a minimum of 3-6 weeks in order for the fat cells to form their own blood supply. After this period it is safe to do all normal activities.

After The Procedure - Your Recovery
- You will be admitted on the day of surgery, and your length of stay depends on which procedure is done. Usually, you will stay in the hospital for about 1 night.
- After the Brazilian Butt Lift you will have tape dressings over your tiny wounds, and tight tape dressings and a compression garment on your tummy / areas where the fat was taken from. On the same day of the surgery, we will encourage you to mobilize gently. Sitting on the edge of your sitting bone will be fine, but you will be encouraged to lie on your side or tummy during the first 2 - 3 weeks of recovery. Avoid full-square sitting on the buttocks for at least 3 - 6 weeks. This is very important because prolonged sitting will damage the fat.
- Post-operative pain in these procedures is easily controlled with oral pain medication.
- You will be mobile from day one and should be back to full exercise within six weeks. No stitches will need to be removed. Most women have approximately 1 week off work, depending on the type of work you do.
- We will keep a close eye on you after your surgery and would normally see you for your follow-up appointments at regular dressing clinics every 2-3 days and then 6 weeks and 3 months following surgery, at which point your post-operative photographs will be taken.

The results of buttock augmentation are immediately visible; you will see your final results 3 months after the surgery, as it takes a while for the fat to “take” and the swelling to settle. What Complications Can Occur?
Patients may experience temporary swelling and bruising following their Kenyan Butt Lift.
Whilst these operations are generally safe in the hands of a qualified and experienced plastic surgeon and regarded to be highly successful, there are, as with any operation, risks involved and the possibility of complications that patients need to consider. Most wound problems are minor and can be managed with simple dressings; however more major wound problems can arise such as infections, asymmetries, fat loss and delayed healing. 10 Things You Can Do After Your Fat Transfer Surgery To Help Ensure More Fat Survives
- WEARING THE RIGHT GARMENTS: Beyond wearing the shaping compression garments that we recommend after surgery, you’ll need to plan to wear clothing that supports your healing process. Squeezing newly transferred fat is the fastest way to kill those cells, so you want to make sure you’re not wearing tight or restrictive clothing during your healing process.
- FEED THE FAT: Your body will need extra calories to sustain the extra work of providing those new fat cells with blood supply and recovering from the surgery. Eating nutrient-dense foods will provide your body with all the vitamins and minerals it needs to support its recovery. Adding in more healthy fats in your diet will be helpful during your recovery. Try adding avocados, salmon, grass-fed butter, almonds, walnuts, extra virgin olive oil, and coconut oil.
- CREATIVE SLEEPING: Squishing your newly transferred fat is a big no-no for at least 2 months after surgery. So you’ll need to get creative about ensuring you’re not laying on your newly transferred fat. For Brazilian Butt Lift, stomach sleeping is a good idea. It’s a little harder for you to roll over from your stomach during the night, so that can be helpful to keep you from rolling onto your backside out of habit. Side sleeping can be an option after BBL Surgery, but only if you feel certain you won’t end up on your back.
- NO DRIVING: Brazilian Butt Lift patients will need to take at least 2 weeks off from driving, and here’s why:
Sitting on your newly transferred fat is going to squish and suffocate those fat cells; but if sitting is bad, driving is much worse. During driving, when you push down on the pedals, you’re pushing back against your butt and lower back. So BBL patients need to be sure not to drive for at least a few weeks after surgery – the closer you can get to 6-8 weeks without driving, the more likely you are to keep a larger percentage of the transferred fat intact. - SIT ON YOUR THIGHS: One trick that helps our patients keep off their butt during the healing process is to sit on a folded towel. Roll up the towel to make a cylinder, then place that towel underneath your thighs as you sit, keeping your weight off your butt. Just be careful not to cut off blood supply (if you’re going numb in your thighs, stand for a few minutes to ensure blood circulation, then re-position).
- NO SMOKING: Smoking is a big no-no after any surgery, because smoking affects the flow of oxygen and nutrients through the blood stream, crippling your body’s ability to heal itself. With fat transfer, your newly transferred blood cells need to re-establish a blood supply in order to survive and thrive in their new home. Smoking makes it harder for your body to build those new blood vessels and affects the quality of blood those fat cells receive during their recovery. Second hand smoke is just as bad for you, so be sure to avoid smoking areas during your recovery, as well.
- EXERCISE STARTING AROUND WEEK 8: Exercise helps to get blood and fluids circulating throughout the body, and sweating is a great way to help your body clear out toxins. You want to avoid strenuous activity during the first 2 months of recovery, to ensure you aren’t burning off any of the transferred fat to fuel workouts. During this time, light walking is helpful, but you should be careful not to burn too many calories, or do any movements that put pressure on your transferred fat. After 8 weeks, your fat transfer should have established proper blood supply, and getting a good sweat session in can help pump fresh, nutrient-rich blood into the area. One tip to remember as you start exercising, is that you want to avoid activities that put a lot of pressure, compression, or trauma on your fat transfer. For example, Brazilian Butt Lift (BBL) patients will want to avoid stationary bike and leg sleds, both of which put a lot of pressure on the buttocks. Instead try the elliptical or hack squat machines. Weight-lifting and other low-impact sports have not been shown to affect the face, so those should be fine to start back up with at 8 weeks.
- STAY HYDRATED: We all know hydration is key to health, but when your body is working hard to repair itself, it relies heavily on water to help clear away waste and move nutrients throughout the body. But drinking water alone is not enough. You need to provide your body with electrolytes, the salts and minerals that help your body hold onto water. Try incorporating a few pinches pink Himalayan salt or a small sports drink (like Gatorade, Powerade, or Smart Water) into your diet each day during the first 8 weeks of your recovery.
- KEEP A STABLE WEIGHT: Weight fluctuations after fat transfer can negatively affect your results. For Brazilian Butt Lift patients, weight gain can cause undesirable effects. Now, just to be clear, we’re not talking about 5 pounds here. However, gaining significant weight after having had liposuction, can cause that fat to accumulate in “new problem areas” – for example, a pear-shaped woman who’s never concerned herself with underarm flab may suddenly find her upper back and arms are now more likely to gain weight first, rather than her tummy (where we’ve removed fat cells during her BBL procedure). Weight loss after BBL may cause your buttocks to lose weight, though it should be proportionate to weight loss throughout the body.
- Maintain optimistic mindset: While the post-op recovery is sometimes difficult and frustrating, it is important to consider the amazing changes your body has undergone. While we know that swelling and irregularities are slow to reduce, after 3 months we will have a look at the before and after pictures and you will be amazed how you have been transformed.
BBL Surgery FAQ
It is very rare for our patients to have to gain weight before a fatgrafting procedure, including KBL. If however, during the consultation, Dr Stasch explains that the fat is not enough for a perfect result, weight gain might be an option to increase the chances meeting your expectations with a good outcome.
It’s an amazingly simple procedure from a scarring point of view: There will be no scars, apart from tiny, 3mm incisions where the lipo cannula goes through the skin. These will be done in well places, hidden areas.
Recovery after Brazilian Butt Lift Surgery is 1 – 2 weeks initially, to recover form the Liposuction. The buttocks are not painful, but sitting is not recommended for 2 -3 weeks, to allow the transplanted fat to “take”, i.e. grow in the transplanted positions. Final results can be expected after 3 months.
The Kenyan Butt Lift (KBL) is essentially the same as the Brazilian Butt Lift, with the exception that Kenyan women have different bodies, expectations and genetics than Brazilian woman. Cultural ideals and aesthetic preferences in Kenya for some patients seeking Buttock enhancement include a “snatched” waist (totally thin waist line), a feminine S-curve, full hips and lateral buttocks, as well as round and perky posteriors. We recommend sleeping on the tummy initially, then slight lateral position with a pillow between the legs and in other strategic positions to avoid pressure on the fat transplanted areas.
We recommend 3 weeks on average to avoid putting pressure on the fat grafted areas. The reason is that the fat cells will survive better when they have good blood supply, which is reduced when lying of them. Special cushions can be used to sit and prevent pressure on the delicate areas. We can provide you with these BBL cushions in case you need them.
Yes that is possible. We are often asked to do tummy tucks as well as Butt Lifts – that’s a special scenario which is still possible, but Dr Stasch will explain in detail the pros and cons of this combination. As long as its safe to combine other ops, it is usually fine.
There are many advantages to having your buttocks and hips enlarged and sculptured using your own tissue and fat. Silicone butt implants are not commonly done, because they can cause many problem such as dislodgement, rupture, and infection. We prefer to use autologous fat in Kenya, using fatgrafting as the safer and better method to give patients who seek this procedure the best results that we can give their bodies.
After 3 months the results will be such that we can take the final “after” picture, since it will show more or less the final result. In some patients, however, the results will even improve further if they maintain their good lifestyle and work on their bodies.
This is an important question that all patients need to be informed about. Dr. Stasch has never had a death after a Kenyan or Brazilian Buttlift, or any other surgical procedure. Worldwide figures show however estimated death rates from fat embolism during or after a Buttlift can be high, but more recent studies will determine the exact incidence of this complication. Most deaths occurred with inexperienced surgeons, or non-plastic surgeons, who would inject the fat into the wrong positions and areas. This would allow fat entering the large blood vessels in the buttock muscles, entering the bloodstream all the way to the lungs.
